Role summary

As a Family Intervention Worker, you will play a key role in delivering intensive support to disabled children and their families at Level 3 need. You’ll work directly with families to strengthen relationships, build confidence, and help parents and carers meet their child’s needs at home and in the community.

 

Using a Think Family approach, you’ll develop and deliver tailored intervention plans, assess needs and strengths, and coordinate support through a multi-agency Team Around the Child/Family model. Working with a high level of autonomy, you’ll manage risk, engage families in complex situations, and drive positive outcomes.

 

You’ll promote inclusion for children with disabilities, deliver targeted support programmes, maintain accurate records, and work collaboratively with partners to improve wellbeing and keep children safe.

 

What you’ll do - key responsibilities

  • Manage a caseload of children, young people and families with complex (Level 3) needs, delivering focused, intensive support in homes, community settings and Children’s Centres, working in partnership to build resilience and independence.
  • Carry out holistic assessments and develop, implement and review outcome-focused support plans, ensuring the Think Family approach and Team Around the Child/Family process are embedded.
  • Coordinate multi-agency support, leading meetings, monitoring progress, and adapting interventions to achieve the best outcomes for families.
  • Identify and manage risks, escalating concerns where needed, and contribute to safeguarding processes including reports and court work.
  • Support inclusion of children and young people with disabilities, using proactive and persistent approaches to drive positive change for families.
  • Build strong relationships with partners and community services, signposting to additional support where appropriate.
  • Maintain accurate records using case management systems and contribute to reports and reviews.
  • Deliver targeted interventions and courses for families and engage in ongoing training and professional development.
  • Work within relevant legislation, policies and professional standards, and take part in supervision, appraisals and team activities.

 

 

What we’re looking for – knowledge, experience & skills

Essential

  • Knowledge and understanding of child development and family relationships
  • Ability to promote equality, diversity, and the interests of service users and carers
  • Commitment to safeguarding and promoting the welfare of children and vulnerable adults
  • Knowledge of external resources, data, and community services to support children and families
  • Some experience of social care (not necessarily in paid employment)
  • Experience of working with children and young people
  • Experience contributing to the induction of new staff
  • GCSEs (A–C / Level 4 or equivalent), including literacy and numeracy
  • Ability to produce clear, accurate written and verbal reports and records
  • Ability to speak fluent English (in line with Part 7 of the Immigration Act 2016)
  • Demonstrates empathy and respect for individual needs
  • Ability to prioritise workload and manage competing demands
  • Strong listening, assessment, and communication skills
  • Ability to build effective professional relationships with clients and colleagues

 

Desirable

  • Broad working knowledge of social care
  • Understanding of the rights of users and carers and the Code of Practice for Social Care Workers
  • Awareness of relevant legislation
  • Knowledge of services and legislation relevant to users and carers’ needs
  • Experience of working as part of a team
  • Clerical or administrative experience
  • Experience of working with children with disabilities
  • Experience of working with children with behavioural or emotional difficulties

 

Additional Information  

  • Permanent, 37 Hour position.
  • Flexible/Hybrid working available, based in Yeovil.  
  • You will need to be able to travel to meet the requirements of this role and be available to work evenings and weekends on a rota basis.
  • 28 day’s annual leave (additional to bank holidays)
